Nothing on this list should keep you from asking for help. If you are missing something, tell us.
- Photo identification, if availableIt helps us confirm who you are. If you do not have one, tell us. It should not stop you from getting help.
- A current medication listInclude prescriptions, over-the-counter medicines, and supplements.
- Relevant health informationRecent test results, records from other providers, or notes about symptoms.
- Questions you'd like to discussWrite them down before you arrive. It is easy to forget once you are in the room.
- Insurance information, if applicableBring your card if you have one. Ask us if you are not sure what to bring.
